We have updated our surplus forms and updated our procedures for submitting surplus forms to our office.
- Please update our NEW forms if you have it saved on your computer.
- Please save our new form to your computer, then type (no handwritten forms will be accepted) into the form save it again then email it directly to us. **Approval Signature is not required, just type in approvals name and extension number. The part that says signature is for our signature.
- Please start emailing all forms to firstname.lastname@example.org instead of faxing. (This is more reliable, plus you get a confirmation that we received it)
